– Proof of income in .pdf, .jpg, or png format (i.e., copy of your personal or business tax return for 2019 or 2020 or audited financial statements for 2019 or 2020)

– A copy of your Maryland State Department of Education (MSDE) license (in .pdf, .jpg, or .png)

– A copy of your Certificate of Good Standing with the State of Maryland (only required for child care centers) in .pdf, .jpg, or .png

– Your completed American Recovery Plan Act Fund Budget Template (Download the Excel version here or the PDF version here, complete, and save.)

To make sure you are fully prepared to start the application, you can download and complete this Sample Application (a fillable PDF). PLEASE NOTE: Applicants for the 2021 Child Care Stability Grant must be able to provide proof of income and attest that they are a Baltimore City-based child care provider and have 20 employees or less. If you are not able to provide the appropriate documentation or attest to the above qualifications, you will not be able to proceed with the application.
